This is a British Sign Language (BSL) version of the COVID-19 vaccination consent form.

The consent form asks for information of a personal nature which we tell you about in this BSL version.

If you have been given a paper version, please take the printed consent form to your vaccination appointment.

Consent does not have to be in writing and therefore you may be asked to give your consent to vaccination either verbally or by non-verbal indication.

If required, at your vaccination appointment you may ask for assistance from a member of NHS staff to help complete the form.
